SubscribeSkywork-VL Reward: An Effective Reward Model for Multimodal Understanding and Reasoning
We propose Skywork-VL Reward, a multimodal reward model that provides reward signals for both multimodal understanding and reasoning tasks. Our technical approach comprises two key components: First, we construct a large-scale multimodal preference dataset that covers a wide range of tasks and scenarios, with responses collected from both standard vision-language models (VLMs) and advanced VLM reasoners. Second, we design a reward model architecture based on Qwen2.5-VL-7B-Instruct, integrating a reward head and applying multi-stage fine-tuning using pairwise ranking loss on pairwise preference data. Experimental evaluations show that Skywork-VL Reward achieves state-of-the-art results on multimodal VL-RewardBench and exhibits competitive performance on the text-only RewardBench benchmark. Furthermore, preference data constructed based on our Skywork-VL Reward proves highly effective for training Mixed Preference Optimization (MPO), leading to significant improvements in multimodal reasoning capabilities. Our results underscore Skywork-VL Reward as a significant advancement toward general-purpose, reliable reward models for multimodal alignment. Our model has been publicly released to promote transparency and reproducibility.
HelpSteer2-Preference: Complementing Ratings with Preferences
Reward models are critical for aligning models to follow instructions, and are typically trained following one of two popular paradigms: Bradley-Terry style or Regression style. However, there is a lack of evidence that either approach is better than the other, when adequately matched for data. This is primarily because these approaches require data collected in different (but incompatible) formats, meaning that adequately matched data is not available in existing public datasets. To tackle this problem, we release preference annotations (designed for Bradley-Terry training) to complement existing ratings (designed for Regression style training) in the HelpSteer2 dataset. To improve data interpretability, preference annotations are accompanied with human-written justifications. Using this data, we conduct the first head-to-head comparison of Bradley-Terry and Regression models when adequately matched for data. Based on insights derived from such a comparison, we propose a novel approach to combine Bradley-Terry and Regression reward modeling. A Llama-3.1-70B-Instruct model tuned with this approach scores 94.1 on RewardBench, emerging top of more than 140 reward models as of 1 Oct 2024. We also demonstrate the effectiveness of this reward model at aligning models to follow instructions in RLHF. Critique-out-Loud Reward Models
Traditionally, reward models used for reinforcement learning from human feedback (RLHF) are trained to directly predict preference scores without leveraging the generation capabilities of the underlying large language model (LLM). This limits the capabilities of reward models as they must reason implicitly about the quality of a response, i.e., preference modeling must be performed in a single forward pass through the model. To enable reward models to reason explicitly about the quality of a response, we introduce Critique-out-Loud (CLoud) reward models. CLoud reward models operate by first generating a natural language critique of the assistant's response that is then used to predict a scalar reward for the quality of the response. We demonstrate the success of CLoud reward models for both Llama-3-8B and 70B base models: compared to classic reward models CLoud reward models improve pairwise preference classification accuracy on RewardBench by 4.65 and 5.84 percentage points for the 8B and 70B base models respectively. Furthermore, CLoud reward models lead to a Pareto improvement for win rate on ArenaHard when used as the scoring model for Best-of-N. Finally, we explore how to exploit the dynamic inference compute capabilities of CLoud reward models by performing self-consistency decoding for reward prediction.
AutoRule: Reasoning Chain-of-thought Extracted Rule-based Rewards Improve Preference Learning
Rule-based rewards offer a promising strategy for improving reinforcement learning from human feedback (RLHF), but current approaches often rely on manual rule engineering. We present AutoRule, a fully automated method for extracting rules from preference feedback and formulating them into rule-based rewards. AutoRule extraction operates in three stages: it leverages a reasoning model to interpret user preferences, identifies candidate rules from the reasoning chain of these interpretations, and synthesizes them into a unified rule set. Leveraging the finalized rule set, we employ language-model verifiers to compute the fraction of rules satisfied by each output, using this metric as an auxiliary reward alongside the learned reward model during policy optimization. Training a Llama-3-8B model with AutoRule results in a 28.6\% relative improvement in length-controlled win rate on AlpacaEval2.0, and a 6.1\% relative gain in second-turn performance on a held-out MT-Bench subset, compared to a GRPO baseline trained with the same learned reward model but without the rule-based auxiliary reward. Our analysis confirms that the extracted rules exhibit good agreement with dataset preference. We find that AutoRule demonstrates reduced reward hacking compared to a learned reward model when run over two episodes. Finally, our case study suggests that the extracted rules capture unique qualities valued in different datasets. Self-Rewarding Language Models
We posit that to achieve superhuman agents, future models require superhuman feedback in order to provide an adequate training signal. Current approaches commonly train reward models from human preferences, which may then be bottlenecked by human performance level, and secondly these separate frozen reward models cannot then learn to improve during LLM training. In this work, we study Self-Rewarding Language Models, where the language model itself is used via LLM-as-a-Judge prompting to provide its own rewards during training. We show that during Iterative DPO training that not only does instruction following ability improve, but also the ability to provide high-quality rewards to itself. Fine-tuning Llama 2 70B on three iterations of our approach yields a model that outperforms many existing systems on the AlpacaEval 2.0 leaderboard, including Claude 2, Gemini Pro, and GPT-4 0613. While only a preliminary study, this work opens the door to the possibility of models that can continually improve in both axes.
CDR: Customizable Density Ratios of Strong-over-weak LLMs for Preference Annotation
Preference tuning of large language models (LLMs) relies on high-quality human preference data, which is often expensive and time-consuming to gather. While existing methods can use trained reward models or proprietary model as judges for preference annotation, they have notable drawbacks: training reward models remain dependent on initial human data, and using proprietary model imposes license restrictions that inhibits commercial usage. In this paper, we introduce customized density ratio (CDR), a training-free and highly effective method that leverages off-the-shelf LLMs for preference data annotation. Our approach uses the log-density ratio between a better-aligned LLM and a less aligned LLM as a reward signal. We explores 221 different LLMs pairs and empirically demonstrate that increasing the performance gap between paired LLMs correlates with better reward generalization. Furthermore, we show that tailoring the density ratio reward function with specific criteria and preference exemplars enhances performance across domains and within target areas. In our experiment using density ratio from a pair of Mistral-7B models, CDR achieves a RewardBench score of 82.6, outperforming the best trained reward functions from same model class and demonstrating competitive performance against SoTA models in Safety (91.0) and Reasoning (88.0) domains. We use CDR to annotate an on-policy preference dataset with which we preference tune Llama-3-8B-Instruct with SimPO. Using reward signals from two relatively weak models, our approach pushes Llama-3-8B to achieve a 37.4% (+15.1%) win rate on ArenaHard and a 40.7% (+17.8%) win rate on Length-Controlled AlpacaEval 2.0, along with a score of 8.0 on MT-Bench.
Skywork-Reward-V2: Scaling Preference Data Curation via Human-AI Synergy
Despite the critical role of reward models (RMs) in reinforcement learning from human feedback (RLHF), current state-of-the-art open RMs perform poorly on most existing evaluation benchmarks, failing to capture the spectrum of nuanced and sophisticated human preferences. Even approaches that incorporate advanced training techniques have not yielded meaningful performance improvements. We hypothesize that this brittleness stems primarily from limitations in preference datasets, which are often narrowly scoped, synthetically labeled, or lack rigorous quality control. To address these challenges, we present a large-scale preference dataset comprising 40 million preference pairs, named SynPref-40M. To enable data curation at scale, we design a human-AI synergistic two-stage pipeline that leverages the complementary strengths of human annotation quality and AI scalability. In this pipeline, humans provide verified annotations, while large language models perform automatic curation based on human guidance. Training on this preference mixture, we introduce Skywork-Reward-V2, a suite of eight reward models ranging from 0.6B to 8B parameters, trained on a carefully curated subset of 26 million preference pairs from SynPref-40M. We demonstrate that Skywork-Reward-V2 is versatile across a wide range of capabilities, including alignment with human preferences, objective correctness, safety, resistance to stylistic biases, and best-of-N scaling, achieving state-of-the-art performance across seven major reward model benchmarks. Ablation studies confirm that the effectiveness of our approach stems not only from data scale but also from high-quality curation. The Skywork-Reward-V2 series represents substantial progress in open reward models, highlighting the untapped potential of existing preference datasets and demonstrating how human-AI curation synergy can unlock significantly higher data quality.
Mutual-Taught for Co-adapting Policy and Reward Models
During the preference optimization of large language models (LLMs), distribution shifts may arise between newly generated model samples and the data used to train the reward model (RM). This shift reduces the efficacy of the RM, which in turn negatively impacts the performance of the policy model (PM). To address this challenge, we propose Mutual-Taught, a self-training method that iteratively improves both the PM and RM without requiring additional human annotation. Our approach mirrors the expectation-maximization (EM) algorithm. In the E-step, the PM is updated using feedback from the current RM, guiding the PM toward a better approximation of the latent optimal preference distribution. In the M-step, we update the RM by constructing training data from the outputs of the PM before and after the E-step update. This process ensures that the RM adapts to the evolving policy distribution. Experimental results demonstrate that this iterative approach leads to consistent improvements in both models. Specifically, our 8B policy model, LLaMA-3-8B-Instruct-MT, achieves a length-controlled win rate of 54.1\% on AlpacaEval-2, while our 8B reward model, FsfairX-LLaMA3-RM-MT, performs on par with GPT-4o-2024-08-06 on RewardBench.
On Designing Effective RL Reward at Training Time for LLM Reasoning
Reward models have been increasingly critical for improving the reasoning capability of LLMs. Existing research has shown that a well-trained reward model can substantially improve model performances at inference time via search. However, the potential of reward models during RL training time still remains largely under-explored. It is currently unclear whether these reward models can provide additional training signals to enhance the reasoning capabilities of LLMs in RL training that uses sparse success rewards, which verify the correctness of solutions. In this work, we evaluate popular reward models for RL training, including the Outcome-supervised Reward Model (ORM) and the Process-supervised Reward Model (PRM), and train a collection of LLMs for math problems using RL by combining these learned rewards with success rewards. Surprisingly, even though these learned reward models have strong inference-time performances, they may NOT help or even hurt RL training, producing worse performances than LLMs trained with the success reward only. Our analysis reveals that an LLM can receive high rewards from some of these reward models by repeating correct but unnecessary reasoning steps, leading to a severe reward hacking issue. Therefore, we introduce two novel reward refinement techniques, including Clipping and Delta. The key idea is to ensure the accumulative reward of any reasoning trajectory is upper-bounded to keep a learned reward model effective without being exploited. We evaluate our techniques with multiple reward models over a set of 1.5B and 7B LLMs on MATH and GSM8K benchmarks and demonstrate that with a carefully designed reward function, RL training without any additional supervised tuning can improve all the evaluated LLMs, including the state-of-the-art 7B LLM Qwen2.5-Math-7B-Instruct on MATH and GSM8K benchmarks.
Skywork R1V2: Multimodal Hybrid Reinforcement Learning for Reasoning
We present Skywork R1V2, a next-generation multimodal reasoning model and a major leap forward from its predecessor, Skywork R1V. At its core, R1V2 introduces a hybrid reinforcement learning paradigm that harmonizes reward-model guidance with rule-based strategies, thereby addressing the long-standing challenge of balancing sophisticated reasoning capabilities with broad generalization. To further enhance training efficiency, we propose the Selective Sample Buffer (SSB) mechanism, which effectively counters the ``Vanishing Advantages'' dilemma inherent in Group Relative Policy Optimization (GRPO) by prioritizing high-value samples throughout the optimization process. Notably, we observe that excessive reinforcement signals can induce visual hallucinations--a phenomenon we systematically monitor and mitigate through calibrated reward thresholds throughout the training process. Empirical results affirm the exceptional capability of R1V2, with benchmark-leading performances such as 62.6 on OlympiadBench, 79.0 on AIME2024, 63.6 on LiveCodeBench, and 74.0 on MMMU. These results underscore R1V2's superiority over existing open-source models and demonstrate significant progress in closing the performance gap with premier proprietary systems, including Gemini 2.5 and OpenAI o4-mini. Temporal Self-Rewarding Language Models: Decoupling Chosen-Rejected via Past-Future
Self-Rewarding Language Models propose an architecture in which the Large Language Models(LLMs) both generates responses and evaluates its own outputs via LLM-as-a-Judge prompting, dynamically improving its generative capabilities through iterative Direct Preference Optimization (DPO). However, our analysis reveals a critical limitation in existing Self-Rewarding paradigms: the synchronized improvement of chosen and rejected responses progressively narrows the representational difference between contrasting samples, undermining effective preference learning. We propose Temporal Self-Rewarding Language Models that strategically coordinate past, present, and future model generations to sustain learning signals. Our dual-phase framework introduces: (1) Anchored Rejection - fixing rejected responses using the past initial model's outputs and (2) Future-Guided Chosen - dynamically curating chosen samples using next-generation model predictions. Extensive experiments across three model families (Llama, Qwen, Mistral) and different model sizes (Llama3B/8B/70B) demonstrate significant improvements when trained with our method compared to Self-Rewarding using same computation resources. For example, Llama3.1-8B reaches a 29.44 win rate on AlpacaEval 2.0 with our method, outperforming the Self-Rewarding baseline (19.69) by 9.75. Notably, our method also demonstrates superior out-of-distribution generalization across mathematical reasoning (GSM8K), knowledge-based QA (ARC, TruthfulQA), and code generation (HumanEval) tasks, even though we do not specifically collect such training data.
SWE-RL: Advancing LLM Reasoning via Reinforcement Learning on Open Software Evolution
The recent DeepSeek-R1 release has demonstrated the immense potential of reinforcement learning (RL) in enhancing the general reasoning capabilities of large language models (LLMs). While DeepSeek-R1 and other follow-up work primarily focus on applying RL to competitive coding and math problems, this paper introduces SWE-RL, the first approach to scale RL-based LLM reasoning for real-world software engineering. Leveraging a lightweight rule-based reward (e.g., the similarity score between ground-truth and LLM-generated solutions), SWE-RL enables LLMs to autonomously recover a developer's reasoning processes and solutions by learning from extensive open-source software evolution data -- the record of a software's entire lifecycle, including its code snapshots, code changes, and events such as issues and pull requests. Trained on top of Llama 3, our resulting reasoning model, Llama3-SWE-RL-70B, achieves a 41.0% solve rate on SWE-bench Verified -- a human-verified collection of real-world GitHub issues. To our knowledge, this is the best performance reported for medium-sized (<100B) LLMs to date, even comparable to leading proprietary LLMs like GPT-4o. Surprisingly, despite performing RL solely on software evolution data, Llama3-SWE-RL has even emerged with generalized reasoning skills. For example, it shows improved results on five out-of-domain tasks, namely, function coding, library use, code reasoning, mathematics, and general language understanding, whereas a supervised-finetuning baseline even leads to performance degradation on average. Overall, SWE-RL opens up a new direction to improve the reasoning capabilities of LLMs through reinforcement learning on massive software engineering data.
Llama-3.1-Sherkala-8B-Chat: An Open Large Language Model for Kazakh
Llama-3.1-Sherkala-8B-Chat, or Sherkala-Chat (8B) for short, is a state-of-the-art instruction-tuned open generative large language model (LLM) designed for Kazakh. Sherkala-Chat (8B) aims to enhance the inclusivity of LLM advancements for Kazakh speakers. Adapted from the LLaMA-3.1-8B model, Sherkala-Chat (8B) is trained on 45.3B tokens across Kazakh, English, Russian, and Turkish. With 8 billion parameters, it demonstrates strong knowledge and reasoning abilities in Kazakh, significantly outperforming existing open Kazakh and multilingual models of similar scale while achieving competitive performance in English. We release Sherkala-Chat (8B) as an open-weight instruction-tuned model and provide a detailed overview of its training, fine-tuning, safety alignment, and evaluation, aiming to advance research and support diverse real-world applications.
Spurious Rewards: Rethinking Training Signals in RLVR
We show that reinforcement learning with verifiable rewards (RLVR) can elicit strong mathematical reasoning in certain models even with spurious rewards that have little, no, or even negative correlation with the correct answer. For example, RLVR improves MATH-500 performance for Qwen2.5-Math-7B in absolute points by 21.4% (random reward), 13.8% (format reward), 24.1% (incorrect label), 26.0% (1-shot RL), and 27.1% (majority voting) -- nearly matching the 29.1% gained with ground truth rewards. However, the spurious rewards that work for Qwen often fail to yield gains with other model families like Llama3 or OLMo2. In particular, we find code reasoning -- thinking in code without actual code execution -- to be a distinctive Qwen2.5-Math behavior that becomes significantly more frequent after RLVR, from 65% to over 90%, even with spurious rewards. Overall, we hypothesize that, given the lack of useful reward signal, RLVR must somehow be surfacing useful reasoning representations learned during pretraining, although the exact mechanism remains a topic for future work. We suggest that future RLVR research should possibly be validated on diverse models rather than a single de facto choice, as we show that it is easy to get significant performance gains on Qwen models even with completely spurious reward signals.
Multimodal LLMs as Customized Reward Models for Text-to-Image Generation
We introduce LLaVA-Reward, an efficient reward model designed to automatically evaluate text-to-image (T2I) generations across multiple perspectives, leveraging pretrained multimodal large language models (MLLMs). Existing MLLM-based approaches require instruction-following data for supervised fine-tuning and evaluate generation quality on analyzing text response, which is time-consuming and difficult to train. To address this problem, we propose LLaVA-Reward, which directly utilizes the hidden states of MLLMs given text-image pairs. To enhance the bidirectional interaction between visual and textual representations in decoder-only MLLMs, we further propose adding a Skip-connection Cross Attention (SkipCA) module. This design enhances text-image correlation reasoning by connecting early-layer visual features with later-layer hidden representations. In addition, LLaVA-Reward supports different types of preference data for efficient fine-tuning, including paired preference data and unpaired data. We train LLaVA-Reward on four evaluation perspectives: text-image alignment, fidelity/artifact, safety, and overall ranking. Empirical results demonstrate that LLaVA-Reward outperforms conventional and MLLM-based methods in generating human-aligned scores for automatic evaluations and inference-time scaling in text-to-image generations.
Skywork-R1V3 Technical Report
We introduce Skywork-R1V3, an advanced, open-source vision-language model (VLM) that pioneers a new approach to visual reasoning. Its key innovation lies in effectively transferring reasoning skills from text-only Large Language Models (LLMs) to visual tasks. The strong performance of Skywork-R1V3 primarily stems from our elaborate post-training RL framework, which effectively activates and enhances the model's reasoning ability, without the need for additional continue pre-training. Through this framework, we further uncover the fundamental role of the connector module in achieving robust cross-modal alignment for multimodal reasoning models. In addition, we introduce a unique indicator of reasoning capability, the entropy of critical reasoning tokens, which has proven highly effective for checkpoint selection during RL training. Skywork-R1V3 achieves state-of-the-art results on MMMU, significantly improving from 64.3% to 76.0%. This performance matches entry-level human capabilities. Remarkably, our RL-powered post-training approach enables even the 38B parameter model to rival top closed-source VLMs. The implementation successfully transfers mathematical reasoning to other subject-related reasoning tasks. We also include an analysis of curriculum learning and reinforcement finetuning strategies, along with a broader discussion on multimodal reasoning. Skywork-R1V3 represents a significant leap in multimodal reasoning, showcasing RL as a powerful engine for advancing open-source VLM capabilities.
AstroMLab 3: Achieving GPT-4o Level Performance in Astronomy with a Specialized 8B-Parameter Large Language Model
AstroSage-Llama-3.1-8B is a domain-specialized natural-language AI assistant tailored for research in astronomy, astrophysics, and cosmology. Trained on the complete collection of astronomy-related arXiv papers from 2007-2024 along with millions of synthetically-generated question-answer pairs and other astronomical literature, AstroSage-Llama-3.1-8B demonstrates remarkable proficiency on a wide range of questions. AstroSage-Llama-3.1-8B scores 80.9% on the AstroMLab-1 benchmark, greatly outperforming all models -- proprietary and open-weight -- in the 8-billion parameter class, and performing on par with GPT-4o. This achievement demonstrates the potential of domain specialization in AI, suggesting that focused training can yield capabilities exceeding those of much larger, general-purpose models. AstroSage-Llama-3.1-8B is freely available, enabling widespread access to advanced AI capabilities for astronomical education and research.
Co-Reward: Self-supervised Reinforcement Learning for Large Language Model Reasoning via Contrastive Agreement
Although reinforcement learning with verifiable rewards (RLVR) shows promise in improving the reasoning ability of large language models (LLMs), the scaling up dilemma remains due to the reliance on human annotated labels especially for complex tasks. Recent alternatives that explore various self-reward signals exhibit the eliciting potential of LLM reasoning, but suffer from the non-negligible collapse issue. Inspired by the success of self-supervised learning, we propose Co-Reward, a novel RL framework that leverages contrastive agreement across semantically analogical questions as a reward basis. Specifically, we construct a similar question for each training sample (without labels) and synthesize their individual surrogate labels through a simple rollout voting, and then the reward is constructed by cross-referring the labels of each question pair to enforce the internal reasoning consistency across analogical inputs. Intuitively, such a self-supervised reward-shaping mechanism increases the difficulty of learning collapse into a trivial solution, and promotes stable reasoning elicitation and improvement through expanding the input sample variants. Empirically, Co-Reward achieves superior performance compared to other self-reward baselines on multiple reasoning benchmarks and LLM series, and reaches or even surpasses ground-truth (GT) labeled reward, with improvements of up to +6.8% on MATH500 over GT reward on Llama-3.2-3B-Instruct. Instruct-SkillMix: A Powerful Pipeline for LLM Instruction Tuning
We introduce Instruct-SkillMix, an automated approach for creating diverse, high quality SFT data. The Instruct-SkillMix pipeline involves two stages, each leveraging an existing powerful LLM: (1) Skill extraction: uses the LLM to extract core "skills" for instruction-following, either from existing datasets, or by directly prompting the model; (2) Data generation: uses the powerful LLM to generate (instruction, response) data that exhibit a randomly chosen pair of these skills. Here, the use of random skill combinations promotes diversity and difficulty. Vanilla SFT (i.e., no PPO, DPO, or RL methods) on data generated from Instruct-SkillMix leads to strong gains on instruction following benchmarks such as AlpacaEval 2.0, MT-Bench, and WildBench. With just 4K examples, LLaMA-3-8B-Base achieves 42.76% length-controlled win rate on AlpacaEval 2.0. To our knowledge, this achieves state-of-the-art performance among all models that have only undergone SFT (no RL methods) and competes with proprietary models such as Claude 3 Opus and LLaMA-3.1-405B-Instruct. Ablation studies also suggest plausible reasons for why creating open instruction-tuning datasets via naive crowd-sourcing has proved difficult. Introducing low quality answers ("shirkers") in 20% of Instruct-SkillMix examples causes performance to plummet, sometimes catastrophically. The Instruct-SkillMix pipeline is flexible and is adaptable to other settings.
Reward Shaping to Mitigate Reward Hacking in RLHF
Reinforcement Learning from Human Feedback (RLHF) is essential for aligning large language models (LLMs) with human values. However, RLHF is susceptible to reward hacking, where the agent exploits flaws in the reward function rather than learning the intended behavior, thus degrading alignment. While reward shaping helps stabilize RLHF and partially mitigate reward hacking, a systematic investigation into shaping techniques and their underlying principles remains lacking. To bridge this gap, we present a comprehensive study of the prevalent reward shaping methods. Our analysis suggests three key design principles: (1) RL reward is ideally bounded, (2) RL benefits from rapid initial growth followed by gradual convergence, and (3) RL reward is best formulated as a function of centered reward. Guided by these insights, we propose Preference As Reward (PAR), a novel approach that leverages the latent preferences embedded within the reward model itself as the signal for reinforcement learning. We evaluated PAR on two base models, Gemma2-2B and Llama3-8B, using two datasets, Ultrafeedback-Binarized and HH-RLHF. Experimental results demonstrate PAR's superior performance over other reward shaping methods. On the AlpacaEval 2.0 benchmark, PAR achieves a win rate at least 5 percentage points higher than competing approaches. Furthermore, PAR exhibits remarkable data efficiency, requiring only a single reference reward for optimal performance, and maintains robustness against reward hacking even after two full epochs of training. OffTopicEval: When Large Language Models Enter the Wrong Chat, Almost Always!
Large Language Model (LLM) safety is one of the most pressing challenges for enabling wide-scale deployment. While most studies and global discussions focus on generic harms, such as models assisting users in harming themselves or others, enterprises face a more fundamental concern: whether LLM-based agents are safe for their intended use case. To address this, we introduce operational safety, defined as an LLM's ability to appropriately accept or refuse user queries when tasked with a specific purpose. We further propose OffTopicEval, an evaluation suite and benchmark for measuring operational safety both in general and within specific agentic use cases. Our evaluations on six model families comprising 20 open-weight LLMs reveal that while performance varies across models, all of them remain highly operationally unsafe. Even the strongest models -- Qwen-3 (235B) with 77.77\% and Mistral (24B) with 79.96\% -- fall far short of reliable operational safety, while GPT models plateau in the 62--73\% range, Phi achieves only mid-level scores (48--70\%), and Gemma and Llama-3 collapse to 39.53\% and 23.84\%, respectively. While operational safety is a core model alignment issue, to suppress these failures, we propose prompt-based steering methods: query grounding (Q-ground) and system-prompt grounding (P-ground), which substantially improve OOD refusal. Q-ground provides consistent gains of up to 23\%, while P-ground delivers even larger boosts, raising Llama-3.3 (70B) by 41\% and Qwen-3 (30B) by 27\%. These results highlight both the urgent need for operational safety interventions and the promise of prompt-based steering as a first step toward more reliable LLM-based agents.
